Bu form, 6698 sayılı Kişisel Verilerin Korunması Kanunu'nun ("KVKK") 11. ve 13. maddeleri ile Veri Sorumlusuna Başvuru Usul ve Esasları Hakkında Tebliğ uyarınca, kişisel verisi işlenen ilgili kişilerin (veri sahiplerinin) haklarını veri sorumlusuna karşı kullanabilmesi amacıyla hazırlanmıştır.

This form has been prepared in accordance with Articles 11 and 13 of the Turkish Personal Data Protection Law No. 6698 (the "KVKK") and the Communiqué on the Procedures and Principles for Applications to the Data Controller, to enable data subjects whose Personal Data is processed by the Provider to exercise their rights.

Oturum açmış kullanıcılar; çoğu hakkı doğrudan Hesap → Gizlilik ve Onay ve Hesap → Veri ve Dışa Aktarma sekmelerinden kullanabilir (rıza geri çekme, veri dışa aktarma, hesap kapatma). Bu form, anonim başvurular veya yazılı/ıslak imzalı talep gerektiren hâller içindir.

Signed-in users can exercise most rights directly from Account → Privacy & Consent and Account → Data & Export (consent withdrawal, data export, account closure). This form is for anonymous applications or where a written / wet-signed request is required.

Eksik veya kimliği teyit edilemeyen başvurular, KVKK'nın 13(2) maddesi ve Tebliğ uyarınca işleme alınamayabilir.Incomplete applications, or applications where the applicant's identity cannot be verified, may not be processed (KVKK Art. 13(2) and the Communiqué).
Üçüncü kişi adına yapılan başvurularda noter onaylı vekâletname (veya muadili yetki belgesi) ibrazı zorunludur.Applications made on behalf of a third party require a notarised power of attorney (or equivalent authority).
Veri sorumlusunun talebi reddetmesi, eksik cevap vermesi veya süresinde cevap vermemesi hâlinde ilgili kişi, cevabı öğrendiği tarihten itibaren 30, herhalde başvuru tarihinden itibaren 60 gün içinde Kişisel Verileri Koruma Kurulu'na şikâyette bulunabilir (KVKK m. 14).Where the Provider rejects the application, gives an incomplete answer, or fails to respond in time, the applicant may lodge a complaint with the KVKK Board within thirty (30) days of becoming aware of the response and, in any event, within sixty (60) days of the application date (KVKK Art. 14).
